    Forms are available online. Please download them, fill them out and email them to our office.

  • We'll Verify Your Insurance

    We’ll verify your insurance based on the information you have provided.

  • Arrive on Time for Your Appointment

    Come to our office at your scheduled date and time.

  • Full X-Rays and Exams

    We’ll thoroughly examine your teeth at your appointment.

  • Teeth Cleanings Will Be Scheduled

    Diagnosis will be taken at the exam and the type of cleaning you need will be determined during this exam.

  • We'll Develop a Treatment Plan

    Sometimes you wont need one, but if you do, we do it during the initial exam!

  • Schedule Your Next Appointment
